Question 1:
Please state your full name. You will later have an opportunity to
list your spouse's name if this document is going to be executed by
him or her also.

If your spouse is going to sign this also, you should answer "yes"
to this question. Although it is not usually necessary to have
both parents sign this document, it is often a good idea and may
eliminate some questions for your guardian later.

Please list your spouse's full name.
Question 4:
A full address is necessary. Start with the number and street of
your dwelling. Put in an apartment number also, if there is one.
Question 5:
List the City or Town where you live.
Question 6:
List the State or Territory where you live.
Question 7:
You should appoint someone you trust to be the guardian of your
child or children.
Question 8:
This is often useful when you are appointing a person with whom
your child or children will be staying. Then appointing the spouse
will allow both of them to look after your child or children.
Question 9:
List the full name of the spouse of your guardian.
Question 10:
List the address of the guardian. Here, you should list the Street
and Number. You should also list an apartment number, if
applicable.
Question 11:
Please list the City or Town where the guardian(s) live.
Question 12:
Please list the State or Territory where the guardian(s) live.
Question 13:
A Child Guardianship Consent Form can include several children when
necessary. If this document is going to include more than one
child, you should answer "yes" to this question.
Question 14:
When the document is only for one child, please list his or her
full name.
Question 15:
If more than one child is to be covered by this document, then you
should list each child's full name.
Question 16:
This is a normal statement, and it will give the guardian those
rights and powers to take care of the child or children. It
includes the right to define medical care as well as to feed,
clothe, shelter and otherwise care for the child or children. If
you do not wish to convey all of these rights the program will
allow you to be more specific, below.
Question 17:
This is very important. Obviously, if you are going away for a
time and can't be contacted, you need to insure that whomever you
are leaving the child or children with has the right to see to it
that they get appropriate medical attention.
Question 18:
Answering "yes" to this question allows the guardian to provide
clothing, food and shelter for the child. Generally this type of
provision is to be used when the child will be with the guardian
overnight.
Question 19:
Answering "yes" to this question, allows the guardian to provide

Question 20:
This allows the guardian(s) to hire a babysitter or put your child
or children in a day care facility.

If you have other special powers that you wish the guardian to
have, you can specify them in this and the following paragraphs.

guardian(s).

This allows you to list alternate and additional powers for the
guardian(s).
